Because insurance policies vary, we can only
estimate your coverage in good faith but cannot guarantee coverage

patient portion must be paid as per the contract agreement.

claim for you. You will be responsible for any balance not taken
care of by your insurance policy. As a courtesy to our patients, we
will bill insurance companies for services provided. If you have any
questions, our knowledgeable staW is always available to answer
them.

Once treatment has started, we will file your
